Welcome to Iowa State University and Iowa State Cyclone Athletics!

 

As per Iowa State University, the Big 12 Conference and NCAA Rules; all student-athletes must satisfactorily complete an athletic pre-participation physical examination within six months prior to participation on any Iowa State athletic team.  There are also a group of forms that will need to be completed and returned to Iowa State Athletics (Insurance Coordinator) prior to the pre-participation physical exam and being cleared to participate.  Participation with any Iowa State athletic team is strictly prohibited until completion of all required medical documentation.  Please choose the appropriate link below for the list of medical requirements you will need to complete prior to participating in athletics at Iowa State University.  Refer to the contact information to the left with any questions related to this pre-participation process.
